I am proud of my course assignments. I spend a lot of time preparing them, and clearly setting forth the expectations in the guidelines. So it is disheartening and somewhat insulting when a student submits work that fails to do what is so clearly asked for.

Simple things, like addressing all the questions, or using a particular format, or proofreading. If you don't take the time to pay attention to the little things, I am already suspicious when I begin grading that you didn't care about the big things, either.

When the amount of time a student puts into an assignment is obviously far less than the amount of time I spent preparing it, bad grades follow. So pay attention to details, and make sure you are following the guidelines.
